Main Duties:

<ul>
<li>Responsible for supporting referrals for the area you are assigned to, carrying-out comprehensive assessments and developing perinatal specialist mental health care plans.</li>

<li>They will monitor and support women during the pregnancy or at post-natal stages, maintain strong links and communication with health providers such as Community Midwives, Specialist Midwives, Health Visitors and other professionals.</li>

<li>Planning care programme approach (CPA) reviews.</li>

<li>Required to attend and cover key functions in the team,- duty, team huddles, team meeting, buddy support/lone working.</li>

<li>As a member of a large team, you will provide support to your colleagues with complex cases, escalating concerns to Safeguarding partners and ensuring that safety plans are in place for high-risk service users.</li>

<li>To keep up to date with specific specialist perinatal research.</li>

<li>Undertake the planning and delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment.</li>

<li>Managing a caseload and the provision of mental health care services to people with mental health care need.</li>

<li>To be responsible for accurate recording of mental health records of all patients on admission, discharge and at review.</li>

<li>To work within the multi-disciplinary team and to form working partnerships with other care providers.</li>

<li>To ensure service users and / or carer(s) are enabled and empowered to make informed decisions about their care and delivery of that care, working with agencies from within and outside the multi-disciplinary team.</li>
</ul>

Requirements of the Social Worker:

<ul>
<li>Degree or equivalent in social work</li>

<li>Current Social Work England registration</li>

<li>Significant frontline experience in Adults Services within the UK</li>
</ul>
