What you will do:

- Undertake assessments, reviews, and safeguarding enquiries in line with the Care Act 2014.
- Use a strength-based approach to empower adults to maintain their independence and achieve their personal goals.
- Collaborate closely with partners to build community resilience and deliver early intervention and preventive support.
- Work with individuals facing complex needs to maximise their potential and support recovery efforts.
- Attend regular team and practice meetings, gaining access to ongoing learning and development opportunities.
- Align with specific postcode areas to manage the triage and support process effectively.
