 What you will do:<ul>
<li>Collaborate with the hospital discharge team and Integrated Care Board to facilitate timely and safe discharges for those with NHS fast track funding.</li><li>Conduct Care Act Assessments, risk assessments, and continuing health care checklists to ensure individuals receive appropriate care and support in the community and local hospices.</li><li>Engage in safeguarding enquiries under Section 42 of the Care Act, ensuring that the rights and safety of individuals are protected.</li><li>Work with individuals, families, and carers in both hospital and community settings to create comprehensive support plans considering Mental Capacity and Human Rights legislation.</li><li>Coordinate Decision Support Tool meetings to evaluate and plan for continuing health care needs.</li></ul>
