The salary for this permanent Mental Health Social Worker job is up to £36,073 per annumMain duties:<ul>
<li>To work as a social worker/care coordinating in managing a caseload of individuals referred into secondary mental health services for support with their mental health needs.</li><li>To complete comprehensive individual needs assessment based on a strengths based approach and recovery principles.</li><li>To co-ordinate appropriate care and support packages of care with clear recovery outcomes. To be accountable for ensuring that the packages are appropriate to the person’s needs and are regularly reviewed.</li><li>To complete or participate in the assessment of risk, and to produce appropriate risk management risks management strategies.</li><li>To lead and participate in the review and evaluation of packages of care and support.</li><li>To liaise with other health and social care professionals, organisations and agencies, including carers and family, housing, probation service, benefits and welfare agencies etc.</li><li>To maintain case records and general administration related to the caseload in line with department guidance and procedures. To work and understand single case file records.</li><li>To ensure that legal literacy and a rights based approach is at the forefront of their work.</li><li>To undertake on occasions the supervision of other health or social care workers, including student supervision.</li><li>To fully participate in the shared responsibilities and tsks of the team/service, including duty systems and service input, and to participate in multi-disciplinary meetings and assessment clinics.</li><li>If an AMHP to participate in the AMHP duty rota and where not an AMHP to be prepared to undertake the training to become so.</li><li>To participate in the team/service and departments staff development programmes and the promotion of specialist skills.</li><li>To carry out such other duties as are required as are commensurate with the grade of the post.</li></ul>
Requirements of this Mental Health Social Worker job:<ul>
<li>A social Work Qualification or equivalent.</li><li>Understanding of relevant legislation.</li><li>Continual professional development.</li><li>Social Work England registered.</li></ul>
