Join Rhodes Wood Hospital in Hatfield as a Social Worker and be part of an environment that has a strong emphasis on teamwork as you change lives for the better.</li><li>You will work 37.5 hours a week alongside a multidisciplinary team at Rhodes Wood Hospital, a specialist CAMHS Eating Disorder service with 3 wards, which empowers recovery in a fast-paced, challenging and fun environment for children and young people aged 8-18 years with eating disorders.</li><li>With experience from the NHS or private sector, you will responsible for assessing the social care needs of young people within a low secure service, which will see you act as a principal provider of quality social work services as you address outlined needs in care plans.&nbsp;</li><li>As a Social Worker, you will manage a dedicated caseload with complex care needs and or psychological conditions, which will see you provide social care assessments, implement focused care work on agreed care plans, facilitate group sessions in collaboration with other disciplines and assisting in the assessment of aftercare placements.</li><li>You will contribute to the MDT care planning process and provide assessment reports to support the completion of young people's CPA documents, which will see you liaise with Children's Services. Departments to ensure that reports and attendance by a representative are available for Mental Health Review Tribunals. Where necessary, you'll be required to prepare reports and attend hearings.</li></ul>

The salary for this permanent Experienced Social Worker job is up to £51,306 per annum. Main duties:<ul>
<li>Manage a caseload within a framework of appropriate reflective supervision, undertaking focused, analytical assessments, SMART plans and reviews that have clear objectives and outcome measures in order to ensure that all vulnerable children and young adults with disabilities receive a service that reflects the Council’s commitment to child and adult safeguarding and meaningful transition into young adulthood.</li><li>Demonstrate and model good practice that reflects a commitment to relationship and strengths-based practice, working collaboratively with disabled children, young people and their families.</li><li>Work directly with disabled children and young people to establish their lived experiences wishes and feelings, using a variety of techniques, as required.</li><li>Maintain accurate and up to date records in line with Data Protection legislation (GDPR) and use Medway Council specific recording systems to promote effective case management.</li><li>Prepare clear, structured and concise reports for meetings, i.e. Child Protection Conferences, strategy meetings, Looked After Children Reviews, Children in Need meetings, and court proceedings as necessary, to the required standard and within the prescribed timescales.</li><li>Keep up to date with research findings, theoretical models and innovative practice within the social work field in order to maintain the highest professional standards.</li><li>Participate in regular reflective supervision with your manager to review casework and critically reflect on, and identify any areas of learning.</li><li>Provide duty cover, on a rota basis, to cover on issues arising and supporting the Multi Agency Safeguarding Hub.</li><li>To participate in one-on-one Performance Development Reviews and contribute to the identification of own and team development needs.</li><li>At the discretion of the Head of Service, such other activities as may from time to time be agreed consistent with the nature of the job described above.</li></ul>
