Bromley's Hospital Social Work Team is at the forefront of delivering safe, strengths-based assessment and timely discharge planning. You will partner with hospital clinicians, community services, and voluntary organisations to promote independence, reduce delays, and ensure the best possible outcomes.

As part of an integrated discharge environment, you will take responsibility for assessing needs under the Care Act 2014, coordinating discharge pathways such as Discharge to Assess (D2A) and Home First, and supporting adults with a range of health and social care challenges.
