What you will do:
- Conduct assessments and dynamic risk assessments.
- Perform Mental Capacity Assessments.
- Operate as a Duty rota worker and manage telephone triage.
- Respond swiftly to crisis and safeguarding concerns with appropriate enquiries.
- Collaborate with a multidisciplinary team to deliver integrated health and social care solutions.
- Utilise exceptional interpersonal skills to communicate effectively and be a supportive team player.
