About the Role:

 The salary for this permanent Social Worker job is up to £52,627 per annum. 
 
 Main duties:

<ul>
<li>To engage and fully participate in supervision and to apply critical reflection and analysis to inform professional decisions. To ensure risks are discussed with your line manager to gain direction, advice and support with practice.</li>

<li>To effectively manage a reasonable caseload appropriate to the level of experience and to maintain adequate throughput of work. To discuss any challenges with caseload management with your line manager in a timely way to access any necessary support with prioritisation.</li>

<li>To be professionally curious when undertaking effective assessments of individual and carer needs and risks within statutory frameworks, using a strengths-based, person-centred approach to ensure they are focused on outcomes.</li>

<li>To work directly in partnership with residents, their families and carers to enable them to maintain their independence and wellbeing, cope with change, attain outcomes they want and need, understand and manage risk, and participate in the life of their communities.</li>

<li>To provide advice and information to residents and carers ensuring that the assessment, care and support plans, information about charging and other information regarding service provision is communicated in a timely way.</li>

<li>To monitor implementation of packages of care and ensure support plans, including funding arrangements for any services, are thoroughly reviewed to meet needs and risks in accordance with statutory requirements and Council policy and deliver highest quality of service and best value.</li>

<li>To take responsibility for consistent, accurate, clear, and timely case recording. To ensure that confidentiality is maintained, and that information is shared with resident, service providers and others agencies (health) in accordance with the Department's data protection and information sharing requirements.</li>

<li>To hold a caseload of higher-level complexity and to be able to conduct legally compliant assessments in a wide range of challenging contexts, cope with difficult conversations and be highly resilient.</li>

<li>To advise and support managers on the relevant matters affecting the service and to contribute as required to change programmes within the service.</li>

<li>To work in ways that develops good working relations and collaborative arrangements with internal and external stakeholders.</li>

 The salary for this permanent Social Worker job is up to £53,194per annum. 
 
 Main duties:

<ul>
<li>To take responsibility for a suitable caseload and develop experience and competency across the following duties with opportunity to progress from one grade to the next when/if progression requirements are met.</li>

<li>Develop and maintain effective relationships with service users, families, carers and communities to help them make informed decisions, enabling them to clarify and express their needs and contribute to service planning that is person-centred and strengths-based.</li>

<li>Work effectively within legal, policy and financial frameworks, obligations, structures and culture, in particular, the Care Act, Menal Capacity Act, Mental Health Act, Human Rights and Equalities legislation and accompanying guidance and codes of practice.</li>

<li>Construct and implement care packages which are clearly linked to the needs of clients, working creatively with community and voluntary organisations to maximise independence, to minimise risks and improve outcomes and the quality of life for service users and their carers.</li>

<li>Be responsible for the assessment and safe discharge of service users from hospital back into community settings, working within challenging timeframes and reducing risk of future readmission.</li>

<li>Undertake Mental Capacity assessments for service users where it is deemed that they may lack the mental capacity to make a specific decision.</li>

<li>Identify service users who are eligible for Continuing Health Care (CHC) or Section 117 funding and undertake checklists and work with other professionals to provide a multi-disciplinary assessment within this framework.</li>

<li>Complete Human Rights Act assessments for those who have no recourse to public funds but may require support under the Care Act.</li>

<li>Take responsibility for managing an individual caseload of clients, regularly monitor, review and evaluate changes in clients needs and ensure that all services contribute effectively to support/care plan, and it remains an effective use of resources.</li>

<li>Intervene in emergency situations to protect vulnerable adults and children and initiate the appropriate statutory or other actions required.</li>

<li>Participate in office duty on a rota basis, responding to new referrals, assessing urgency of requests and implementing actions to meet immediate needs.</li>

<li>Maintain and update case notes and other records within required timescales, write reports provided written responses as required, if required, give evidence in court in relation to care or other proceedings.</li>

<li>Develop and maintain up to date knowledge and competency and personal effectiveness in relevant legislation, social work practice and changes in social work including strengths-based working delivery models through development as agreed by the Team Manager.</li>

<li>Fully participate in individual supervision and performance appraisal as well as participate in the development and implementation of performance indicators and quality service standards, always striving for improved performance both personally and as a service.</li>
</ul>
