The role operates on a hybrid basis, with 2 to 3 days working in the office each week.

What you will do:

  • Manage a complex caseload of children looked after and care leavers, ensuring their needs are effectively supported.
  • Develop and review personalised care plans to achieve positive outcomes for children and young people.
  • Work collaboratively with partner agencies and professionals to coordinate effective support packages.
  • Advocate for children and young people, ensuring their voices and needs are represented.
  • Maintain accurate records, assessments, and reports in line with statutory requirements.
  • Build positive relationships with children, families, carers, and professionals to promote better outcomes.
