What you will do
• Assessment and Planning: Complete comprehensive assessments, risk assessments, and personalised recovery plans for individuals entering the service.
• Case Management: Manage a caseload of service users, ensuring timely access to treatment, interventions, and ongoing support.
• Psychosocial Interventions: Deliver one-to-one support sessions and facilitate group-based interventions where required.
• Criminal Justice Support: Assist individuals transitioning from custody into community-based treatment services.
• Multi-Agency Working: Work closely with Probation Services, Courts, Prisons, Police, healthcare providers, and community organisations.
• Advocacy: Support service users in accessing housing, healthcare, employment, education, and welfare services.
• Harm Reduction: Promote harm reduction approaches to minimise substance-related risks and reduce reoffending.
• Record Keeping: Maintain accurate, confidential, and compliant case records in accordance with organisational policies.
• Outcome Monitoring: Monitor recovery progress, evaluate outcomes, and adjust support plans as appropriate.
