What you will do:

• Assess the needs of individuals and families affected by domestic abuse and identify appropriate support pathways.
• Develop and implement personalised support plans in partnership with service users and relevant agencies.
• Work collaboratively with community partners and multidisciplinary teams to improve outcomes.
• Advocate for clients’ rights and coordinate multi-agency meetings to ensure effective support.
• Provide emotional support, practical guidance, and safety planning to empower clients.
• Maintain accurate and up-to-date case records in line with statutory requirements and organisational policies.
• Participate in team meetings and contribute to the continuous improvement of services.
