We are seeking an enthusiastic Advanced Practitioner to join the Community Review Team and the Residential Review Team for a 3 month full-time period. This locum position offers flexibility and variety in your work, as you engage with both community and residential settings. If approved, you may even have the chance to support the South West Older People Team, ensuring your experience is diverse and rewarding.

What you will do:
- Conduct assessments and reviews for individuals in community and residential settings, ensuring care plans meet their needs and are delivered in a person-centred manner.
- Collaborate with multiple teams, including the Community Review Team, Residential Review Team, and potentially the South West Older People Team, to enhance service delivery and outcomes.
- Lead and support care staff, providing supervision and professional guidance to maintain high standards of practice.
- Work closely with families, carers, and other professionals to provide comprehensive support and achieve positive outcomes for individuals.
- Record and report on your assessments and interventions in line with organisational and legislative requirements.
- Stay informed of changes in legislation and best practices to ensure continuous improvement in service delivery.
