We are seeking an enthusiastic Advanced Practitioner to join the Community Review Team and the Residential Review Team for a 3 month full-time period. This locum position offers flexibility and variety in your work, as you engage with both community and residential settings. What you will do:
- Conduct assessments and reviews for individuals in community and residential settings, ensuring care plans meet their needs and are delivered in a person-centred manner.
- Collaborate with multiple teams, including the Community Review Team, Residential Review Team, and potentially the South West Older People Team, to enhance service delivery and outcomes.
- Lead and support care staff, providing supervision and professional guidance to maintain high standards of practice.
- Work closely with families, carers, and other professionals to provide comprehensive support and achieve positive outcomes for individuals.
- Record and report on your assessments and interventions in line with organisational and legislative requirements.
- Stay informed of changes in legislation and best practices to ensure continuous improvement in service delivery.
