What you will do
• Duty Responsibilities: Participate in the daily duty rota, responding to incoming referrals, enquiries, and urgent requests for support.
• Assessments: Conduct strengths-based assessments under the Care Act 2014 to identify eligible needs and support requirements.
• Care Planning: Develop, implement, and review person-centred care and support plans, including applications for funding where required.
• Section 117 Work: Collaborate closely with health colleagues to develop and review Section 117 aftercare plans.
• Hospital Discharge: Attend multidisciplinary team meetings and contribute to effective discharge planning from inpatient settings.
• Safeguarding: Lead safeguarding enquiries as a Lead Enquiry Officer, ensuring appropriate interventions and outcomes.
• Carers Assessments: Complete Carers Assessments and provide support to carers in line with statutory requirements.
• Mental Capacity Act Assessments: Undertake Mental Capacity Assessments and Best Interest decisions where required.
• Partnership Working: Build strong working relationships with health partners, including CNWL and other community services.
