Join Assessment and Discharge Team in Adult Social Care We're recruiting a qualified social worker to work in our established Community Hospital and Community Support Bed Assessment and Discharge Team. About You We're looking for a care home experienced manager with strong leadership skills and experience with multidisciplinary teams and conducting risk assessments . You'll also either have a Registered Managers Award Level 4 , or Leadership and Management in Care level 4/5 , or willingness to study for one. And, above all, whenever you communicate with a resident or co-worker, you'll be truly considerate and kind.
