What you will do:

• Complete Care Act assessments in accordance with statutory duties and regulations.
• Undertake safeguarding adults work and manage Section 42 enquiries diligently.
• Carry out Mental Capacity Assessments ensuring the application of the Mental Capacity Act in practice.
• Perform strengths-based reviews and formulate support plans for service users.
• Conduct home visits in Newcastle and surrounding areas as necessary.
• Collaborate effectively with health professionals and external partner agencies.
• Maintain accurate case records and timely documentation.
• Manage a diverse caseload, ensuring compliance with statutory timescales.
