Social Worker - Mental Health Team
Full-Time, Substantial post qualifying experience, HCPC Registered, Diploma in Social Work or equivalent Degree.

As a Social Worker responsible for working effectively with service users/providers and other parties within the Community Mental Health team (CMHT) you will manage a defined case load. The Community Mental Health Team consists of joint health- and social care-funded operational team managers, senior practitioners, social workers, social work assistants and CPNs. The team supports service users from the age of 18.
You will complete assessments, statutory reviews and mental capacity assessments, and attend multi-disciplinary meetings and Community Hospital ward rounds. Throughout, you will complete safeguarding investigations and carers' assessments, and provide advice and information. Collaboration is key, and the team works alongside other health partners, police and voluntary organisations.
This role reports to the Team Manager.
About the client
Our client is a forward thinking Council based in West London. The role in the Children's and Families department is focused on the delivery of services relating to vulnerable children, young people and complex families. These services will be delivered against specifications set by the People Commissioning function in line with the County Council's overarching outcomes.
Promote the principles of person centred practice in social work service delivery
Uphold appropriate targeting of resources and the principle of minimum intervention through joint working with partners in universal services
Utilise responsibility as delegated at the discretion of the Locality Social Work Manager for representing the Social Work service within the locality, in any management arrangements for jointly managed services
Implement, manage and report on performance management using the Social Work performance management and devolved budgetary reporting frameworks
Support the Community and Locality Social Work Manager in providing inputs to the service evaluation and review of the effectiveness and the resource implications of existing service plans and new initiatives
Contribute to local stakeholder liaison and community planning agendas as directed by the Locality Social Work Manager
What we need from you
Degree or equivalent in social work
Current HCPC registration
Significant senior level frontline experience within children's services
2 years post qualified with experience within local authority Community Mental Health Teams
