We are looking for a Care Coordinator to work the following:
Week one - Monday collect the on call phone at 4pm and hold overnight until 9am Tuesday morning Tuesday from 4pm hold the on call phone overnight until 9am Wednesday morning .
Week two - Monday and Tuesday on call then Friday on call at 4pm until Monday morning 9am - to be part of the on call team
Having the on call phone will include going out on care rounds to cover unallocated care or sickness/holidays.
To receive handover from Care Managers in the office
To cover care rounds where necessary
To answer all phone calls from carers service users and families
To organise care rounds if a shift becomes outstanding
To work in a team every other weekend to cover the service working alongside the Care Managers
To be computer literate
To be creative diverse and be able to work unsociable hours
This role would suit an experienced senior carer starting out on a management pathway
Good communication and listening skills
NVQ level 3 desirable
On call phone £50.00 per evening and per day at the weekend
All care is paid at senior carer rate which we have different levels of pay for.
Pay to be discussed at interview
The job can be more hours if needed to work in the office to cover holidays or sickness or if they wanted to do care rounds to up the hour.
