An exciting opportunity has arisen for an experienced nurse to work within our Primary Care Mental Health Team at HMP Pentonville. As a dynamic, creative, team player you'll become part of a multidisciplinary service. You'll be responsible for the continuing management of patients referred to Primary Care Mental Health, including assessment of care needs, the development, implementation and evaluation of programmes of care, health promotion activity and care of vulnerable offenders. You'll support the delivery of care for services users throughout their time at HMP Pentonville.

What do we need from you so you can do the job?
As a Mental Health Nurse / Social Worker / Counsellor or Psychotherapist you will be an integral part of the healthcare team assessing all referrals to Primary Care Mental Health and ensure continuity of care through appropriate communication. You'll attend the Health and Wellbeing multi-disciplinary team meeting on a daily basis to receive referrals and feedback ongoing care of patients on caseload and contribute to ACCT reviews and assessments when required. As the Primary Care Mental Health Practitioner you'll also co-facilitate psycho-educational groups such as anxiety, mood management and sleep hygiene. Ideally you'll have a qualification in/or knowledge of Cognitive Behavioural Therapy.

Do not worry... you will be supported and mentored by experienced teams of highly skilled nurses and practitioners as well as management.
