Social Care Team Leader with experience working within disability services required for a service based in Limerick. This role is a 12 month fixed term contract; 35 hours per week.
This role requires the successful candidate to work directly with the person, their family and support staff and use resources available to them, to develop valued social roles.
Main Duties and Responsibilities
Establish and maintain the right relationships and collaborative partnerships with the person, their family, the staff teams and other stakeholders
Work directly with the person in their own home and community providing support with all aspects of daily activities and in implementing their personal plan. This involves using social role valorization theory and a supported self-directed living as a framework for practice.
Support each person to effectively communicate, to makes decisions, exercise choice, control and autonomy in all aspects of their life and in self directing their support arrangement.
Effectively coordinate the supports provided in line with each person’s changing needs and manage and resolve issues as they arise.
Lead, manage and supervise the staff teams in delivering a quality service.
Manage the administrative, health, safety and HR requirements within the support arrangement in line with regulation, standards and best practice.
Contribute to and monitor the outcomes for each person as identified in their personal plan.
Experience in working as a Team Leader
Excellent facilitation, leadership and communication skills
Strong reflective practice and clear theory of practice in supporting people with a disability
Relevant third level qualification
