Job Description

We are seeking a capable and experienced Band 6 Care Co-ordinator to join the Community Adult Learning Disabilities service on the beautiful Island of Guernsey, in the Channel Islands.
This opportunity is open to either a Registered Nurse or Allied Health Professional, with LD experience and appropriate statutory professional registration.

In supporting the Team Manager you will:
- Provide care co-ordination for an allocated caseload of adults Service Users with learning disabilities and/or autism.
- Conduct assessments of needs for adults with Service Users, as well as Carer Assessment when indicated.
- Manage a complex caseload as well as co-working with other relevant professionals
- Act as a lead professional within the Team to ensure co-ordination of support for Service Users who have complex care management needs.

Please note; Guernsey has adopted the AfCbanding system, but have applied their own salary scale.
The current Guernsey Band 6 salary is up to £47,094 plus an annual bonus of £1,286 and higher unsocial hours enhancements; 36% evening/Saturday and 72% Sunday/BH.
Annual leave starts at 36 days up to 42 days with full credit for NHS service for annual leave and sick leave

Person requirements :
Registered Nurse or Allied Health Profesional with full registration with the NMC or HCPC.
Post-graduate Learning Disabilities and/or Autism experience at senior Band 5 or Band 6-equivalent level.
Experience of undertaking assessments, care planning and co-ordinating care or support
To hold a driving licence.
