Service Area: Adult Community Services

The States of Guernsey are seeking to appoint a Care Co-Ordinator to join the team at Adult Community Services. The successful post holder will provide care co-ordination for an allocated caseload of adults with learning disabilities and/or autism.

  • Band 6 - £34,763 - £41,956 BAR £43,327 - £47,094 per annum

 Relevant Health or Social Care Qualification which includes registration with a professional body.


1. Relevant post qualification experience with adults with learning disabilities and/or autism.

2. Experience of undertaking assessments, care planning and co-ordinating care or support for adults with learning disability or autism.

3. Good presentation skills, especially with regard to presenting information at panels and meetings.

4. Ability to communicate clearly and appropriately at all levels, both orally and in writing, with the ability to develop and maintain good working relationships with people of all levels.

5. Committed to promoting anti-discriminatory practice and equal opportunities, with an ability to demonstrate an understanding of how equality and diversity can be promoted.

6. Able to develop good working relationships with service users, carers and other professionals.

7. Ability to operate as a member of a team, working in partnership with colleagues and contributing to the development of services through a team approach.

8. Ability to make sound judgements and decisions with confidence and to cope in stressful situations and provide calm leadership.

9. Experience of developing learning and development opportunities and facilitating training.

10. To be in possession of a clean and current driving licence.
