Job Description

We are Manchester Local Care Organisation- an exciting, new public sector organisation that is bringing together NHS community health services and social care services in the city. We are looking for an experienced and motivated B3 Senior Health Care Support Workers to join our dynamic Community based MCR service in south Manchester

Manchester Community Response Service includes Discharge to Assess ( supporting patients on discharge from hospital) and Crisis Response ( supporting people in their own homes to avoid an unnecessary hospital admission) we also include Intermediate Care team , Reablement, Community Flow and Community IV teams in the service . HCA staff work flexibly across the D2A & Crisis teams

This is an exciting time to be part of our new integrated multi disciplinary and multi-agency teams supporting patients to return home from hospital or community beds, and providing relevant assessment of needs once they are discharged home

Our team operates as a clinically led multi-disciplinary team comprising of Clinical Lead, Nurses, Occupational therapists, Physiotherapists, , support workers, pharmacists and social care colleagues.

The service is a 7 day, extended hours 08:00 - 22:00 provision working closely with home based rehabilitation and care support

As well as bringing your experience and drive to the team you will be encouraged to learn & develop in your new role via regular supervision & support.

You will be adaptable, committed, well organised and able to work well under pressure. You must be able to travel around various community locations delivering care.

Trust policy requires that the cost of submitting & processing the successful applicant/s DBS application be recovered via salary deduction following start in post. The amount of £29.50(standard disclosure) or £47.50 (enhanced disclosure) Which will be deducted from salary, in the first 3 months of employment. If you wish to withdraw once a DBS application has commenced you will be still liable for payment.
