Job Description

The successful candidate must have completed the Diploma in Health and Social Care Level 3 or equivalent. Candidates must also have literacy and numeracy level 2 or equivalent and must complete the Trust Health Care Assistant competencies within the first year. Internal staff must have completed the full framework before they can apply.

We are seeking individuals whoare motivated, flexible, and able to communicate effectively and have a commitment to providing a Person Centred delivery of services and, to the involvement of service users, family members and carers in all aspects of service delivery. It is essential that the post holder possesses a good clinical knowledge base, has experience within Learning Disability settings and should have an awareness of the Mental Health, Mental Capacity Act, and is also motivated and can demonstrate innovation and forward thinking.

To support Transforming Care implementation the post holderswillbe part of an existing Secure Outreach Services which operate within Tees Esk & Wear Valleys NHS Foundation Trust which support people to safely transition into the communityfrom secure settings.

Applicants must have a flexible approach to their work as they will be expected to take part in the on-call rota and should be able to travel independently to various locations within the Trust on a regular basis.

The post holder will work in a supportive environment that is committed to personal and professional development.

This post requires the post holder to frequently travel to various locations within the Trust. A lease car scheme is in operation
