Job Description

We are looking for an experienced Mental Health Practitioner who has an interest in perinatal mental health to join a newly developing, dynamic and innovative team in the role of Care Coordinator.

This post is open to any professional with an appropriate registration (RMN, OT or SW)

You will have significant experience of post qualification experience at Band 6 or equivalent and demonstrate passion around working alongside women and their families with complex mental health needs during the perinatal period.

The service covers BaNES, Wiltshire and Swindon. We work on a "Hub and Spoke" model where the hub is in Chippenham (where the team meeting is held on a Tuesday) and spoke sites are in Swindon, Salisbury and Paulton BaNES. Whilst you will work in an identified part of the patch, cross area working may be required. This post will be based in Chippenham to primarily cover the northern half of Wiltshire/parts of BaNES.

Please be aware this post requires significant amounts of travel at times.

This is a 9 Month Fixed Term/Secondment opportunity to cover Maternity Leave. If you currently work for AWP please make sure you have the support of your line manager before applying.
