Job Description

An opportunity has arisen for a suitably qualified and experienced Registered Professional Social Worker, to join an integrated health and social care Community Mental Health Team for Adults covering a 7 day a week working pattern.
The successful candidate will be required to fulfil the role of a Community Mental Health Practitioner / Care coordinator as part of a multi-disciplinary team. The team covers the North area of Manchester and operates from McCartney House in Harpurhey .
You will be responsible for the management of a caseload of clients with complex mental health needs and associated social care issues, for which risk assessment and risk management are essential requirements. The successful candidate will be expected to take part in the team duty on a rota basis.
