You will hold and manage a caseload of people as a case manager. The team covers an ethnically and culturally diverse area, and you will have the opportunity to work with clients from a wide range of backgrounds, and with a wide range of psychosocial needs.
By assessing your clients and their carers needs you will be able to access and sign post to a wide range of community resources and provider services available in the local area.
You will work with clients to identify goals as part of overall care, and establish and evaluate appropriate treatment programs for service users within the team.
In future and under the guidance of senior staff the post holder will be expected to participate in providing quality first line psychosocial assessments as part of GMMH's Functional CMHT's duty role and work proactively in forging professional working relationships with multi-agencies.
These posts will prepare the post holders to develop the skills and experiences in preparation to apply for band 6 practitioner posts within the community
