An exciting opportunity has arisen to join the Community Learning Disability Health Team in North Lincolnshire, based at Meridian House.We are looking to recruit 3 Senior Support Workers to support across the Nursing, Allied Health Professional and Psychiatry services to promote health and wellbeing.
We are looking for highly motivated individuals with the relevant experience to help provide care coordination for service users, using knowledge of different legislation such as the Mental Health Act, Mental Capacity Act 2005 and the Care Act.
You will be expected to carry out a range of duties to support the various professionals in the team to meet the needs of those with learning disabilities across the North Lincolnshire locality, so transport is essential.
The post holder will participate in the delivery of skilled care to patients within their own home and will carry a caseload of delegated patients.
You will provide effective; evidence based general and specific care for patients under the indirect or remote supervision of registered practitioners.
Plan, organise and deliver clinical care as agreed with the supervising practitioner.
Use personal effectiveness and knowledge to contribute to the development and delivery of a quality service.
Ensure a high standard of care for all patients by effective communication with patients, carers and the wider health care team
You will work with Primary and Acute services, Local Authority, Provider services and Advocacy to name a few, therefore communication skills are essential.
This is an excellent opportunity for someone who is seeking to broaden their range of skills and knowledge in caring for people in their own environment. Relevant health care experience within a community setting is essential and previous experience of working with people who have learning disabilities would be advantageous.
You will need to be a team player with excellent communication skills and able to work in conjunction with other professionals.
