Job Description

The Rotherham Community Learning disability Team is a specialist Health Care team working to meet the specialist health care needs of adults with learning disabilities. We are looking to recruit a motivated and enthusiastic practitioner into this challenging and rewarding post.
You will be responsible for assessing and supporting the health needs of adults with a learning disability.
The purpose of this post is to carry out.
  • Comprehensive Continuing Health Care Assessments.
  • Monitoring and reviewing of care packages/treatment interventions.
  • Caseload management
The minimum requirements
  • Relevant 1st level nursing qualification Registered Nurse Learning Disabilities.
  • Appropriate demonstrable post registration experience.
  • Knowledge of current legislation and statutory policy frameworks associated with community care.
  • Ability to communicate with a wide range of individuals and groups.
  • Highly motivated and conscientious.
  • Experience of care planning for complex health care needs.
  • Ability to work flexibly to meet the demands of the Service.
You need to be a good team player and be able to demonstrate a good understanding of current evidence based practice, have access to transport and the ability to travel within the required geographical area.
