  • Medium secure services on the St Bernard's Hospital site in Ealing
  • Services for people with complex and severe personality disorder at the Cassel Hospital in Richmond
  • High secure services at Broadmoor Hospital in Berkshire

West London Mental Health NHS Trust has established innovative primary care services across the London Boroughs of Ealing, Hounslow, and Hammersmith and Fulham. The aim is to provide direct advice and support to GPs, and enable adults with chronic but stable mental health conditions to access voluntary and statutory services to promote and maintain their recovery.
As a Primary Care Mental Health Practitioner you will work closely with GPs
and other primary care professionals in the Hammersmith and Fulham Borough to deliver evidenced based interventions, support discharges from secondary to primary care, and when appropriate, prevent people being admitted to secondary care. You will provide a consultation and advice service to the whole primary care team (including training and specialist mental health knowledge). A key role is to tackle the stigma experienced by adults with mental health issues.
We are looking for a highly skilled and committed professional to join our team to deliver effective and efficient interventions that enhance recovery, prevent non-essential routine referrals to secondary care, and support the primary care team.
The successful applicants will be mental health professionals and expressions of interest from social workers and O.T.'s are welcomed. You must have experience of working in the community as a clinician and managing a complex case load. You will need to understand recovery approaches and how they can be implemented.
