An opportunity has arisen for a community support worker, to join a vibrant integrated health and social care Community Mental Health Team for Adults.
The team covers the Wythenshawe area of Manchester and operates from the Brian Hore Unit in Withington.
The post holder will work alongside the Community Mental Health Team in South Manchester under the supervision of the team manager, undertaking physical health checks, physical health clinics and general health and wellbeing assessments. The post holder will assess physiological observations and promote healthy lifestyles for our service users. The post holder will assist in enabling service users to access the support they need to recover from their mental health and physical health problems, promoting recovery in order to return to everyday living. They will also assist care co-coordinators in working across boundaries of care to plan, implement and evaluate care within care pathways as part of the stepped care model.
