This exciting ongoing locum opportunity offers full-time hours, working Monday to Thursday from 9:00am to 5:00pm and Friday from 9:00am to 4:30pm. This specialist role focuses on supporting adults and young people transitioning from Children's Services, undertaking complex assessments, and ensuring individuals receive the most appropriate care, support, and placement arrangements. What you will do:

• Conduct Care Act Assessments for adults aged 18 and over, including those transitioning from Children's Services into Adult Services.
• Complete Mental Capacity Assessments and make Best Interest decisions in accordance with the Mental Capacity Act 2005.
• Undertake Community Deprivation of Liberty Safeguards (DoLS) assessments and associated reviews.
• Complete Continuing Healthcare (CHC) Checklists and Decision Support Tools (DSTs).
• Apply strengths-based approaches and Least Restrictive Practice principles to all areas of assessment and care planning.
• Support social care assessors and colleagues with complex cases, prioritisation, and case management.
• Collaborate effectively with health professionals, care providers, voluntary organisations, and other statutory partners through a multidisciplinary approach.
