What you will do:

  • Undertake the role of Duty Practitioner, responding effectively to urgent and routine referrals.
  • Complete comprehensive initial assessments of children, young people, and their families.
  • Carry out risk assessments and ongoing risk management to promote safe and effective care.
  • Develop evidence-based case formulations and person-centred care plans.
  • Manage a varied caseload, ensuring high-quality, timely interventions.
  • Fulfil safeguarding responsibilities in line with local and national guidance.
  • Deliver community-based mental health interventions across a range of settings.
  • Participate in multidisciplinary team (MDT) meetings to support integrated care planning.
  • Attend child protection conferences, Team Around the Child (TAC) meetings, and other multi-agency forums as required.
