What you will do:

  • Undertake assessments under the Care Act 2014 for adults and carers, ensuring that practice is person-centred and focused on outcomes.
  • Develop, implement, and review care and support plans designed to promote independence and overall wellbeing.
  • Manage a varied caseload, including complex cases, ensuring all activities and documents are recorded accurately and timely, following council policies.
  • Lead safeguarding enquiries (Section 42), conduct risk assessments, and make best interest decisions in accordance with the Mental Capacity Act.
  • Collaborate effectively as part of a multi-disciplinary team, maintaining strong communication lines with NHS colleagues, housing, and voluntary sector partners.

Person Specification:

  • Holding a Degree in Social Work or equivalent such as DipSW, you are registered with Social Work England.
  • Demonstrate a sound understanding of the Care Act 2014, Mental Capacity Act 2005, safeguarding legislation, and the principles of Making Safeguarding Personal.
  • You will possess an enhanced DBS clearance, essential for this role to ensure the safety and security of the vulnerable adults you serve.
