The salary for this Social Worker job is up to £51,574 per annum. 
 
 Main duties:

<ul>
<li>To take responsibility for a suitable caseload and develop and develop experience and competency across the following duties with opportunity to progress from one grade to the next when/of progression requirements are met.</li>

<li>Develop and maintain effective relationships with service users, families, carers and communities to help them make informed decisions, enabling them to clarify and express their needs and contribute to service planning that is person centred, and strengths based.</li>

<li>Work effectively within legal, policy and financial frameworks, obligations, structures and culture particularly, the Care Act, Mental Capacity Act, Mental Health Act, Human Rights and Equalities legislation and accompanying guidance and codes of practice.</li>

<li>Construct and implement care packages which are clearly linked to the needs of clients, working creatively with community and voluntary organisations to maximise independence, to minimise risks and improve outcomes and the quality of life for service users and their carers.</li>

<li>Be responsible for the assessment and safe discharge of service users from hospital back into community settings, working within challenging timeframes and reducing risk of future re admission.</li>

<li>Undertake a range of Care Act Assessments and Section 42 enquiries through the safeguarding process, in accordance with relevant statutory requirements, identifying risks, needs and options, including self-directed support options.</li>

<li>Undertake Mental Capacity assessments for service users where it is deemed that they may lack the mental capacity to make a specific decision.</li>

<li>Identify service users who are eligible for Continuing Health Care (CHC) or Section 117 funding and undertake checklists and work with other professionals to provide a multi-disciplinary assessment within this framework.</li>

<li>Complete Human Rights Act assessments for those who have no recourse to public funds but may require support under the Care Act.</li>

<li>Take responsibility for managing an individual caseload of clients, regularly monitor, review and evaluate changes in clients needs and ensure that all services contribute effectively to the support/care plan, and it remains an effective use of resources.</li>

<li>Undertake ongoing and urgent case reviews as necessary.</li>

<li>Intervene in emergency situations to protect vulnerable adults and children and initiate the appropriate statutory or other actions required.</li>

<li>Participate in office duty on a rota basis, responding to new referrals, assessing urgency of requests and implementing actions to meet immediate needs.</li>

<li>Maintain and update case notes and other records within required timescales, write reports provided written responses as required, if required, give evidence in court in relation to care or other proceedings.</li>

<li>Develop and maintain up to date relevant knowledge and competency and personal effectiveness in relevant legislation, social work practice and changes in social work including strengths based working delivery models through development as agreed by the Team Manager.</li>

<li>Work effectively as a member of a team and contribute through team meetings, forums and organisational events to the development of the service.</li>
</ul>

Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for an Adult Social Worker within the Safeguarding Service to work full time based in Worcestershire.The salary for this permanent Social Worker job is up to £44,771 per annum. Main duties:<ul>
<li>To work with people to make them safer and support them to manage risks.</li><li>To work in partnership and engage other professionals to ensure there is a multi-agency approach to safeguarding.</li><li>To meet statutory requirements including best practice in relation to the principles of the Care Act 2014 and the Mental Capacity Act 2005 and the Mental Health Act 1983.</li><li>As an Experienced Social Worker, it is a requirement to be autonomous in the application of relevant legislation, policy, procedures and social work theory.</li><li>To support the professional and practice development of safeguarding social workers and safeguarding support officers and other Adult Social Care staff through reflective practice, peer support, mentoring and buddy system.</li><li>To support and promote opportunities for students within the team, including shadowing.</li><li>To gather information and make a decision on whether the S42(1) Care Act 2014 criteria are met and triage accordingly.</li><li>To put in place alternative actions to prevent future risk where the criteria are not me.</li><li>To undertake safeguarding enquiries assessed as low risk.</li><li>The Social Worker may be expected, on occasions, to represent the Authority in court and also may be required to attend as a witness or give evidence.</li><li>To represent the service, adults and carers views at a range of meetings and exchange information, make decisions and review progress.</li><li>To build to excellent and effective relationships with other agencies and the local community to sign-post when appropriate.</li><li>To promote the welfare of children and act on any concerns regarding the safety of children.</li><li>The Social Worker will undertake duty responsibilities in line with the requirements of the team.</li><li>To prepare and participate in reflective practice and supervision.</li><li>The Social Worker must meet the continuing professional development requirements of the professional regulator and keep abreast of legislative changes.</li><li>To maintain personal and professional development to meet the changing demands of the job, participate in appropriate training activities and encourage and support staff in their development and training.</li></ul>
