Based at Hundens Land in Darlington, you will work full-time within a collaborative environment of Social Workers and community assessment officers, linking closely with an in-house Reablement Service.

What you will do:

- Collaborate with a team of Social Workers and community assessment officers to facilitate hospital discharges

- Conduct necessary assessments of individuals' care and support needs post-discharge

- Travel to hospitals, care homes, and service users' homes within the Darlington area

- Complete support planning and safeguarding enquiries as needed

- Apply Mental Capacity Act consideration and recording

- Evaluate post-discharge progress to ensure effective reablement and recovery

- Embrace a "Home First" approach promoting independence and well-being

- Report directly to the Team Manager maintaining high standards of practice and service delivery

Requirements of the Social Worker:

- Degree or equivalent in social work

- Current registration with Social Work England

- Extensive frontline experience in social services within the UK as a qualified Social Worker
