What you will do:

* Care Act 2014 Assessments & Reviews: Conduct first-time adult care and support assessments in line with statutory requirements.

* Mental Capacity Act (MCA) & Best Interest Decisions: Carry out mental capacity assessments, attend Best Interest meetings, and support with DOLS/COP applications.

* Person-Centred Support Planning: Collaborate with individuals, families, and multidisciplinary teams to produce tailored care plans.

* Risk and Safeguarding Management: Assess and manage risks, implement safeguarding plans, and conduct Section 42 enquiries.

* Transition Coordination: Liaise closely with children's social care, SEND, education, NHS, housing, and other agencies to ensure seamless handovers.

* Funding & Continuing Healthcare: Explore and advise on funding options, such as Continuing Healthcare and Section 117.

* Caseload & Duty Management: Maintain a reflective caseload and participate in duty/triage rotations.

* Review Meetings: Engage in IRO meetings, EHCP reviews, Best Interest meetings, and other multi-agency collaborations.
