Social Worker - Transfer of Care Team in Southwark, earning £32 hourly. What you will do:

* Deliver comprehensive social work services, managing complex cases in the Adult Social Care team

* Conduct in-person, complex assessments and discharge planning for hospital patients

* Undertake mental capacity assessments and facilitate discharge planning meetings with medical colleagues

* Provide support to families and liaise with medical and social care professionals

* Mentor and coach new social workers, helping them to develop best practices

* Collaboratively work with the entire team to manage risk and promote positive outcomes for service users
