What You Will Do:

Undertake Care Act assessments to support adults' needs in the community.

Carry out Mental Capacity Act assessments and make Best Interest decisions.

Lead and support on safeguarding enquiries , advocating for vulnerable individuals.

Attend and contribute to multidisciplinary and Best Interest meetings .

Collaborate with external agencies to develop comprehensive, person-centred care plans .

Accurately maintain records and reports, ensuring accountability and compliance.

Offer guidance and support to service users and families , promoting independence and well-being.
