Social Work Advance Practitioner within Adults Community Team in Barnsley UK earning £37 hourly for a full-time position. What you will do:

- Conduct Care Act Assessments, incorporating critical frameworks such as the Mental Capacity Act and DOLS.

- Support the team in managing Continuing Health Care Assessments.

- Provide supervision and guidance while leading reflective practices and complex decision-making processes.

- Handle complex cases, promoting independence and early intervention while ensuring personalisation and self-directed support.

- Liaise with individuals, families, and multi-disciplinary partners, making informed decisions and planning effective care packages.

- Update and maintain electronic and paper-based systems with precision and confidentiality.

- Deliver professional advice and guidance, ensuring that best practices and statutory frameworks are consistently applied.

- Facilitate training sessions and contribute to continuous professional development efforts within the team.

- In-depth knowledge of the legal frameworks influencing Adult Social Care.

- Experience in supporting teams, managing complex decisions, and leading reflective supervision.
