What you will do:

- Collaborate with the AMHP Hub, working alongside a colleague to conduct assessments under the Mental Health Act 1983, supporting vulnerable adults.

- Develop and implement care plans tailored to individual needs, ensuring the highest quality of service delivery.

- Liaise with multidisciplinary teams to coordinate care and ensure seamless service provision.

- Maintain detailed and accurate records in accordance with legal and professional standards.
