Your Responsibilities

Conduct Care Act-compliant assessments to determine support needs and strengths.

Manage a diverse caseload, including safeguarding matters, promoting independence and well-being.

Work collaboratively with families, carers, health professionals, and community organisations.

Provide advice, address safeguarding concerns, and connect individuals with appropriate services.

Apply a solution-focused approach to guide decision-making and care planning.

Ensure accurate and timely documentation in line with statutory requirements and best practice.

Candidate Requirements

SWE registration and a recognised Social Work qualification

Minimum of two years' post-qualification experience

Confidence in undertaking mental capacity assessments and safeguarding work

A proactive, community-focused mindset with excellent interpersonal skills
