Key Responsibilities

Carry out initial assessments to determine the needs, strengths, and risks associated with children and families.

Engage directly with children and families to develop informed and effective care and support plans.

Work collaboratively within a multi-agency Duty & Assessment Team to ensure coordinated and high-quality service delivery.

Maintain accurate, detailed, and timely records in line with statutory and organisational requirements.

Participate actively in team meetings, training, and development activities to continually enhance practice and outcomes.
