What you will do:
* Provide valuable advice and information to service users and their families.
* Set clear, attainable goals for reablement and rehabilitation.
* Conduct Care Act assessments and craft person-centred care and support plans.
* Facilitate reviews and carry out Mental Capacity Assessments and Best Interests meetings.
* Signpost effectively to community resources available to those in need.
* Work closely with individuals with diverse health and social care needs, including informal carers.
* Engage in critical safeguarding activities, including initial fact-finding and risk assessment, ensuring safety and well-being.
