Social Worker Locum Opportunity within the Young Adults (16 - 25) All Age Disability Team in Worcestershire . Duration: 7th May 2025 to 6th August 2025. Earn a competitive hourly salary of £32.67 - £63.50 (including weekends and holidays) , working full time from 09:00 to 17:00, 37 hours per week. The All Age Disability Team supports the transition of young individuals from Children's Services into their adult lives, working in areas such as education, employment, and independent living.

What you will do:

- Support young people aged 16-25 with learning disabilities, physical disabilities, or complex health needs in their transition to adulthood.

- Collaborate with Worcestershire Children's Services, Adult Services, SEND, health colleagues, schools, and colleges to provide a holistic range of support services.

- Utilise Liquidlogic effectively in both adult and children's systems to ensure seamless service provision.

- Engage in person-centred planning to prepare young adults for challenges in education, employment, and independent living.

- Drive to various locations within Worcestershire and occasionally nationally to deliver services, making a tangible difference in young lives.
