What you will do:
- Implement and manage pathway planning for children and young people in the care system, ensuring each has a clear and supportive plan tailored to their individual needs.
- Conduct regular keeping in touch visits and maintain accurate records of these interactions.
- Use Liquid Logic to update and manage case records, ensuring all information is current and accessible.
- Attend monthly case supervision sessions, engaging in reflective practices to continually enhance service provision.
- Collaborate closely with team members and other stakeholders, including participation in reflective group supervision.
- Assess risk and coordinate support around care leavers, working to mitigate potential challenges they may face.
- Support team managers by offering insights and expertise to advance pathway planning initiatives for separated young people.
- Drive to various locations as needed within the role, requiring a full driving license and access to a vehicle with business insurance.
