What you will do:

* Conduct statutory Child and Family assessments in accordance with CA 89 and the Chronically Sick and Disabled Act.

* Develop and chair the implementation of Child in Need and Child Protection plans as required.

* Engage effectively with children and young people facing communication challenges.

* Evaluate risks to children with disabilities and propose sound protection measures.

* Identify suitable short break services for children with disabilities.

* Manage Children in Care, ensuring care plans are comprehensive and current.

* Conduct court work when necessary, ensuring the best outcomes for the children and their families.

* Facilitate the transition for young adults into adult services, crafting Transition Plans that meet their needs.
