What you will do:

* Review and assess incoming referrals and contacts within the MASH to determine appropriate pathways and responses.

* Conduct threshold conversations and home visits to engage children, families, and carers, ensuring their safety and support.

* Collaborate closely with team members, including social workers, health professionals, education specialists, and police officers.

* Develop and maintain comprehensive records, reports, and documentation to facilitate case management.

* Provide informed recommendations and case presentations to management teams for effective service delivery.

* Participate in multi-disciplinary meetings, interfacing with various agencies to coordinate efforts and enhance service outcomes.
