What you will do:

* Conduct assessments and provide targeted interventions for children with disabilities, ensuring their needs are met in line with current legislation and the best practice framework.

* Work collaboratively with parents, carers, and multi-agency teams to design and implement care plans that support children's development and welfare.

* Advocate for children's rights and safeguard their wellbeing through diligent case management.

* Maintain accurate records and reports, providing clear documentation for case reviews and audits.

* Offer advice and support to families, helping them navigate social services effectively.

We have two Social Worker vacancies in the Disabled Children's Team at Stockton. Applicants must provide at least two detailed practice-based references using the agency's standard reference template before engagement. We require candidates with three years of post-qualifying experience. The completed declaration form attached should be returned by candidates' agency upon application.
