Job Description
Job Title: Social Worker
Location: Dublin
Hours: 35 hours per week
Reporting to: Senior Medical Social Worker

Role Purpose:
As a Social Worker, you will provide essential emotional and practical support to patients across all wards. Working in close collaboration with a multidisciplinary team, you'll play a key role in guiding patients through their hospital journey, offering counselling, discharge planning, and advocacy services to ensure they have the resources and support needed for their recovery.
Key Responsibilities:
* Patient Support: Deliver compassionate counselling, information, and support services tailored to the needs of patients and their families.
* Social Work Services: Conduct thorough assessments, assist with discharge planning, and connect patients with community resources to ensure a seamless transition from hospital to home.
* Multidisciplinary Collaboration: Act as a consultant to our healthcare team on the psychosocial aspects of patient care, arranging case conferences and working closely with medical, nursing, and allied health professionals.
* Community Liaison: Partner with community health services and other agencies to guarantee continuous care and support for patients beyond their hospital stay.
* Professional Development: Engage in ongoing professional education, maintain required registrations, and attend workshops and conferences to stay informed on best practices.
* Communication: Keep accurate and detailed patient records, share relevant information with the healthcare team, and contribute positively in team meetings to support a collaborative environment.
Qualifications:
* Essential: National Qualification in Social Work (NQSW)
* CORU registration.
* Ideally have Medical Social Work experience but not essential
* Skills: Proficiency with IT systems, including Windows and Office software.
