Job Description

Empowering customers to live rewarding and happy lives. That’s when it hits home.

We have a fabulous opportunity for you to join our growing and brilliant team based in Ipswich, supporting our customers who all have learning disabilities and/or autism who are coming to us through the transforming care pathway in Suffolk. Our new service has been designed to provide an emergency response which pro-actively prevents people from being admitted to hospital in a crisis.

We support up to 4 customers who are experiencing behavioural and/or mental health crisis within our CQC registered specialist Admission Avoidance service. We also offer support to customers in the community or in other care settings throughout Suffolk as an Urgent Outreach service. We have a brand new, purpose-built site which has been designed to consider the needs of anyone who may need to stay with us. The new building is stunning; it has its own private car park, the building is fitted with state of the art assistive technology, air conditioning, it has its own gardens and it is built on a huge field so we will have loads of space outside! Each customer has their own, very spacious self-contained flat plus large communal areas to socialise. Although customers don’t stay with us for long, we want them to feel relaxed, safe and comfortable whilst they are with us.

We’re a very busy service, supporting our customers who are experiencing crisis to achieve their goals and aspirations using their bespoke, positive behaviour support plans. We work in partnership with a wide variety of social care and health professionals to aid an individual’s recovery. You’ll provide a variety of holistic support, so no two days are the same! You could be supporting someone to go to college or work, maybe going to music lessons or a football match, to arts and crafts, even just going walking in the countryside, anything that they enjoy that is engaging and therapeutic to their recovery. You could even find yourself enjoying a Netflix binge, going for a walk or a trip to the swimming pool.

If you’re a pro at supporting people through difficult times and who present with behaviours of concern, able to identify and act upon safeguarding issues, love to be busy every day and go home knowing you’ve made a positive change to someone’s life then this is the job for you! You’ll constantly learn new things and be stretched professionally, emotionally, physically and practically; your learning will be immense.

About you

You’re passionate and motivated to help vulnerable people make positive and lasting changes to their lives. You’ll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds.

You’ll help change our customer’s lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews. Working alongside Support Coordinators and the wider team, you’ll be delivering support against the customers support plans. Each plan is designed to meet our customer’s individual needs and aspirations, whilst providing personal care in a dignified manner.

You’ll be supporting customers who need 1:1 or 2:1 support 24/7 who are in crisis – so although we expect you to have lots of brilliant times with our customers, we also know that some days will be challenging. Therefore, it’s important to us that you have had experience in this sector, working with customers with high level support needs and experience of managing behaviours that challenge. We also need you to be able to escalate concerns to shift leaders and managers and be able to respond quickly and thoughtfully to changing situations. It’s a fast-paced job for sure!

To make sure that we keep our records straight, and customers safe, it's really important that you are technologically savvy (or at the very least not a technophobe!)

To make sure our customers are well supported, it would be amazing if you have worked in an adult CQC regulated service before and supported people with Learning Disabilities and / or Autism. You may have experience in hospitals, secure services, or care homes. Don’t worry, we won’t drop you in the deep end, we’ll help to you understand how things work at Home Group! Ideally but not essential you will have knowledge and experience of working with customers in crisis and a working knowledge of Positive Behaviour Support principles. We’ll support you to complete your care certificate as part of your probationary period if you don’t have this already!

As this is also community-based role, you need to be able to drive, as well as having access to a car insured for business purposes. Don’t worry, we’ll pay you expenses to cover this use!

Working hours

We provide 24-hour support to our customers every day and night. You’ll work on a rota basis and take your turn working days, evenings, weekends, and bank holidays.
